CNA / PCA JOB DESCRIPTIONS

POSITION TITLE: Certified Nursing Assistant/Personal Care Assistant

DEPARTMENT: Home Care

REPORTS TO: Supervisor/ Manager

POSITION SUMMARY:

Certified Nursing Assistant/ Personal Care Assistant is a member of the Home Care team who works under the direct supervision of the supervisor and follows the client’s plan of care. The Personal Care Assistant/ Certified Nursing Assistant is responsible for reporting any issues/concerns regarding the client’s personal care needs, changes in the client’s condition, and/or any observations that may affect/impact the client’s general health.

OVERVIEW:

Certified Nursing Assistant/Personal Care Assistant is required to assist with a combination of basic personal care activities or home care services. Clients requiring these services are unable to care for themselves or function on their own independently due to age, frailty, medical disability or no family support due to certain circumstances.

Personal Care Tasks
(Allow independence and give personal choices)
Bathing (tub, shower, or bed).
Routine skin care (topicals as prescribed).
Oral care (including dentures).
Grooming and shampooing hair (topicals as prescribed).

Assisting with attire/dressing.
Assisting with toileting/hygiene.

Ambulation and Transfers:
(Person assist or mechanical lift as needed)
Assist in/out of bed, and wheelchair.
Assisting with walking.
Encouraging physical activity.

Nutrition:
(follow meal plans/diet or restrictions as needed)
Preparing meals.

Encouraging proper nutrition.
Assisting with eating.

Housekeeping:
(These tasks are ONLY for areas used by the client)
Vacuuming and/or sweeping.

Dusting, and Sweeping/Mopping.
Emptying trash, and Laundry / Bed linen.

Cleaning Kitchen and Washing dishes.
Cleaning bath/bedroom.
Cleaning refrigerator (for outdated foods).

Cleaning common areas.

Medically Related Activities:
(provides assistance as needed)
Observing and reporting changes in the client’s condition.
Picking up prescription drugs.
Accompanying clients on medical appointments (as needed).
Reminding the client to take medication(s).

Home Management:
(providing assistance as needed)
Grocery shopping (in person/online).
Bill paying.
Written/online applications.

Scheduling personal or medical appts.
Contact family/friends/emergency.
Other/Errands (as needed).

Qualifications:
Experience: Previous experience as a caregiver or in a similar role is preferred. At least 5 years of experience.
Skills: Excellent communication and interpersonal skills, empathy, and patience.
Certifications: Certification as a nursing assistant (CNA) is required. CPR and First Aid certification preferred.
Physical Requirements: Ability to perform physical tasks such as lifting, bending, and standing for extended periods.

Background Check: Must pass a background check and have a clean driving record.
